Root Canal Treatment: When Do You Actually Need a Root Canal?
A toothache does not always mean you need a root canal. Sometimes a simple filling is enough, while in other cases the infection may have reached the nerve of the tooth and Root Canal Treatment (RCT) may be needed.

When Do You Need a Root Canal?
A root canal may be recommended when the pulp (nerve) inside the tooth becomes severely inflamed or infected.
Common warning signs include:
Severe or persistent toothache
Pain that keeps you awake at night
Prolonged sensitivity to hot or cold
Pain while biting or chewing
Swelling around the tooth or gum
A pimple or pus discharge on the gum
A tooth that becomes dark or discoloured
A tooth that has suffered significant trauma
However, not every toothache requires an RCT. Tooth sensitivity, a cavity, a cracked tooth, gum problems or a high filling can also cause pain. A proper examination is needed to identify the actual cause.
Does a Tooth Always Hurt When It Needs an RCT?
No.
Sometimes a tooth can have an infection without causing significant pain. An infection around the root may even be discovered during an X-ray taken for another reason.
So, waiting for severe pain is not always a good idea.
What Happens During Root Canal Treatment?
During RCT, the infected or damaged pulp is removed from inside the tooth. The root canals are then cleaned, disinfected, shaped and sealed.
The tooth is finally restored with an appropriate filling or, when required, a crown to protect it.
The number of appointments depends on the tooth and the complexity of the treatment.
Is Root Canal Treatment Painful?
Modern root canal treatment is performed under local anaesthesia, so most patients should not experience significant pain during the procedure.
Some tenderness or discomfort for a few days after treatment can occur. If pain is severe, increasing or accompanied by swelling, contact your dentist.
What Should You Do After an RCT?
After the appointment:
Avoid chewing until the numbness has completely worn off.
Avoid very hot food or drinks while you are numb.
Follow your dentist's medication instructions.
Continue brushing and cleaning your teeth normally, while being gentle around the treated area.
While Waiting for Your Permanent Crown
If your tooth has a temporary filling or temporary crown, avoid chewing hard or sticky foods on that side until the permanent restoration is placed.
Avoid things such as:
Ice
Hard candies
Very hard nuts
Extremely crunchy foods
Biting bones
Using your teeth to open packets or bottles
Whenever possible, chew on the opposite side.
A root-treated tooth that has not yet received its final restoration can be more vulnerable to fracture, so protecting it between appointments is important.
After Your Permanent Crown Is Cemented
Once your permanent crown is placed:
Follow the eating instructions given by your dentist.
Avoid very sticky foods for the initial period if advised.
Avoid deliberately biting extremely hard objects.
Continue brushing and flossing normally.
If your bite feels high or uncomfortable, contact your dentist for a review.
When Should You Contact Your Dentist?
Get in touch with your dentist if you develop:
Severe or increasing pain
Significant swelling
Persistent discomfort
A temporary filling or crown comes out
Your bite feels unusually high after the final restoration
Any symptom that concerns you
Can Root Canal Treatment Save Your Natural Tooth?
In many cases, yes. When a tooth can be predictably treated and restored, RCT can allow you to keep your natural tooth instead of extracting it.
However, not every tooth can be saved. Your dentist will assess the amount of remaining tooth structure, the condition of the roots and surrounding tissues before recommending the most appropriate treatment.
